RN Case Manager

Responsibilities:

As an RN Case Manager, you will case manage home healthcare clients, within a structured territory caseload with limited travel. You will be familiar with federal and state regulations. You will plan, organize and direct homecare services for our medical and/or behavioral clients.

Qualifications:

An RN Case Manager will need to have strong interpersonal skills, strong organizational skills and be able to handle multiple tasks. You will be a registered nurse with a current Connecticut license and a valid Connecticut driver’s license.
